Preparing to Go Home - Primary Caregiver
Going home is a different kind of hard. As the primary caregiver, you will be balancing the care that is provided by professionals who work shifts. It is critical that you ask for and receive help, and that you make processes to simplify your life.

Before you leave the hospital checklist:
- Printed discharge instructions (2 copies). This needs to include WHAT to look for, WHEN to call, and WHO to call for medical help.
- Read the printed discharge instructions and make sure you understand them. If not, don't leave yet. Ask for help.
- A list of prescription medicines and either the pharmacy of where to pick them up OR the actual medicines.
- A nutrition plan/guidelines.
- Any durable medical equipment (wheelchair, oxygen, etc) OR the names and numbers of who will deliver these things to your home.
- A primary person who can come and help you get settled tomorrow - a friend or family member.
